Non-Custodial Parent Request - Motion for Relief To be filled out by non-custodial parents when they are requesting the OCSS file a Motion for Relief on their behalf. This is the form in English. PDF file, less than 1

Self-Represented Litigant - Motion for Relief This is used when the non-custodial wants to file a motion for relief, but the OCSS is unable to assist. The completed form should be presented to the Court. The Court will assign a hearing date for your Motion for Relief. PDF file, less than 1
