Electronic Payment Card (EPC) Enrollment Form For the custodial parent to complete when they are choosing to receive their child support payments on a Kids Card or EPC. PDF file, less than 1

Ending Direct Deposit Required to be completed and returned to the RIOCSS when a Custodial Parent has decided to receive their child support payments via an Electronic Payment Card (EPC), known as a Kids Card. PDF file, less than 1

Self-Represented Litigant - Motion for Relief This is used when the non-custodial wants to file a motion for relief, but the OCSS is unable to assist. The completed form should be presented to the Court. The Court will assign a hearing date for your Motion for Relief. PDF file, less than 1
